FirefighterStory.com | Effective immediately upon signature | Legally binding under the E-SIGN Act
This agreement is between the individual submitting content ("Contributor") and FirefighterStory.com and its operators ("Site"). By signing below, the Contributor agrees to the following terms.
Contributor certifies that no submitted content contains Protected Health Information (PHI) as defined under the Health Insurance Portability and Accountability Act (HIPAA). This includes but is not limited to: real patient names, dates of birth, medical record numbers, Social Security numbers, addresses, phone numbers, photographs, or any other information that could identify a real patient. Any violation of HIPAA law arising from submitted content is the sole responsibility of the Contributor.
All names of persons, patients, crew members, bystanders, supervisors, and any other individuals mentioned must be entirely fictional or sufficiently altered so that no real person can be identified. All city names, county names, fire department names, station numbers, and geographic identifiers must likewise be fictional or altered. The Contributor affirms this has been done prior to submission.
Contributor releases FirefighterStory.com, its owners, operators, administrators, and affiliates from any and all claims, actions, damages, losses, or liability — including legal fees — arising from or related to the submitted content, its publication, distribution, or republication in any form, including printed compilations.
Contributor accepts full and sole personal legal responsibility for the content of any submission. If any submitted content is found to contain real PHI, defamatory statements, or otherwise violates any applicable law, the Contributor — not FirefighterStory.com — shall be solely liable. FirefighterStory.com reserves the right to remove any content at any time without notice.
By typing their name in the signature field below and submitting this form, the Contributor agrees that this constitutes a valid and legally binding electronic signature under the Electronic Signatures in Global and National Commerce Act (E-SIGN Act, 15 U.S.C. § 7001), carrying the same legal weight as a handwritten signature.
